Helps people with disabilities, who are living in a nursing home or other institutionalized living, to move into the community


Services can include:
* Information about what needs and resources are available
* Relocation coordination services to plan and organize the move and service transition
* Financial resources and information, help to plan benefits and funding
* Medical insurance and benefits coordination
* Help to find an accessible place to live
* Help to set up services
* Transportation planning
* Help to plan household needs, like furniture

Eligibility

People with disabilities who live in a nursing home or an Intermediate Care Facility for Persons with Developmental Disabilities (ICF/DD) and want to move into the community into a less-restrictive setting, like an apartment or supportive housing.
