Developmental Disabilities Case Management helps to coordinate services for assistance and supervision for people with developmental disabilities to live in and function well in communities, including:

Case management is a collaborative process to plan, evaluate, coordinate services and advocate for the individual. The programs focus is to identify the individuals unique needs and to minimize the impact of the disability on the persons life while assuring services and support for the person.
