Application Instructions
Walk-in appointments are available every Tuesday and Thursday from 1:00pm - 5:00pm. To schedule an appointment at the Health Resource Center, call (763) 450-3385 Ext: 5100.
To receive services, a completed and signed consent form is needed annually from the parent or guardian. Consent forms are available online, in school health offices and at the Health Resource Center.
