Program that works on behalf of individuals and/or groups to make sure that they receive the benefits and services for which they are eligible
Additional Information
Provide services to Hmong seniors within Hennepin county to become self-sufficient and independence at the same time reduce isolation.
Provides information and referrals to Hmong Seniors for:
* Housing
* Medical and financial services, including senior housing, Medicare/Medicaid, social security benefits, food stamps, and energy assistance
Additional services include:
* Person-centered case management services
* Transportation for shopping, medical appointments, or other activities
* Social and educational program activities
* Volunteer companions
Bilingual staff are also able to provide interpretation and translation services for Hmong Seniors.
