Description
Services for children or adults who live in their own home that provide support in the following community living service categories:
* Community participation
* Healthy, safety and wellness
* Household management
* Adaptive skills
Support includes:
* Cueing
* Skill maintenance
* Guidance
* Instruction
* Incidental assistance with activities of daily living
* Assistance with coordination of community living activities.
There are three types of Individualized Home Supports (IHS):
* Without training
* With training
* With family training
IHS can be provided in the person's own home or in community spaces used by the general public, and either in person or remotely.

Available to children and adults on Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI) Waiver and Developmental Disabilities (DD) Waiver.
ADDITIONAL INFORMATION:
Individuals who are Deaf, DeafBlind, Hard of Hearing, Blind, and Low Vision including transition students between the ages of 14 - 22 and adults from 18 - 70.
