Outpatient health clinic offering preventative, diagnostic (exam or test) and treatment services


Some clinics are certified as Health Care Homes, where primary care providers, families and patients work together to support wellness and improve health outcomes. Intake
Staff will start by getting to know the patient, asking them questions - preferably with a loved one / caregiver present. Staff will review their current symptoms and their dementia risk assessment results, and conduct cognitive and other screenings.


Chart Review
The clinician will review all medical information from the intake and other providers either electronically or on paper.


Evaluation/Care Plan
The clinician will start by getting to know the patient, asking them several questions - preferably with a loved one or caregiver present. The patient's current symptoms and any issues, caregiving status, medications, and results from the intake will be reviewed. Additional screenings and a brief physical exam will be performed. A care plan will be written that outlines the findings, patient goals, and recommended steps. Recommendations for additional tests or referrals for specific care may be suggested.


Ongoing Care
Clinic staff may contact the patient in person or by phone approximately four or more times per year. Onsite and outreach education will be offered for caregivers, friends, and families.
