Case managers help people to plan, access, evaluate and coordinate services from their service providers. Plans are customized to each person's own needs and preferences.

* Advocacy, information, assistance, program development and coordination of services
Provides outreach on many topics including:
* Advance directives
* Scams, fraud, identity theft
* Housing resources
* Matter of Balance classes
* Chronic Disease Self-Management
* Advanced care planning
* Health insurance counseling (Medicare Parts A, B, C and D)
* Long Term Care Insurance and information
* Alzheimer's information and support group
* Caregiver support groups
* Nutrition Assistance Program for Seniors (NAPS)
