Description
A community clinic that is certified to offer comprehensive, person-centered and family-centered, trauma-informed, evidence-based behavioral health and substance use disorder services in a one-stop-shop model.
Provides a variety of services to children or adults with complex mental health and substance use disorders so that they do not have to piece together care across multiple providers, increasing the likelihood that care is received before a person enters into a crisis.
Core services provide:

* Care coordination with other providers and systems
* Screening, assessment, diagnosis and risk assessment
* Outpatient mental health and substance use disorder services
* Targeted case management
* Outpatient primary care screening and monitoring
* Community based mental health care for veterans
* Peer, family support and counselor services
* Person-centered and family-centered treatment planning
* Psychiatric rehabilitation services
