Comprehensive review of all current medicines by a licensed pharmacist to help get the most benefit for your health and lifestyle.
Review includes all prescription medications, herbal supplements, and over-the-counter medicines to help you understand:
* If you are taking only the medicines and amounts you need
* How your medicines interact with each other
* If all your medicines are safe and effective
* If there are any lower-cost options
Review will also answer questions and provide education on your medicines.

ADDITIONAL INFORMATION:
Anyone can use this service. Although this service is provided by a pharmacist, you do NOT have to be a patient to receive this service. The pharmacist can work with your health care providers, no matter where you receive health care.
You may be eligible for medication management services at a reduced cost, in your home or at a Hennepin Healthcare clinic, if you:
* Are 60 years or older
* Live in Anoka, Carver, Dakota, Hennepin, Ramsey, Scott or Washington counties

* A contribution is requested from all participants based on income level if not covered by insurance. However, no one will be turned away for inability to pay.
