Honoring Choices Minnesota promotes end-of-life care planning.
Health Care Directive forms are available to download in Arabic, Chinese, English, Hmong, Russian, Somali, Spanish and Vietnamese.
Education guides are available to help provide more information about the wishes outlined in the health care directive and terminology used.
Certified advance care planning staff are available to provide information, documents, answer questions and can assist in properly completing your health care directive.
Staff have scheduled times to be available at community locations in the Central MN area and by virtual zoom sessions to anyone in Minnesota.
