Adult day services provide an individualized and coordinated program of activities. Services are directed at maintaining or improving a person's ability to self-care. This includes health, social and nutritional services. Services are directed toward the achievement of specific outcomes identified after assessment in a service plan. A person cannot receive adult day services for more than 12 hours in one 24-hour period.

People who use adult day services require some supervision. They often need assistance with personal care and other activities of daily living such as transfer, mobility or dressing. Some providers may offer a bath.
Adult day service is provided in adult day service centers licensed by the Minnesota Department of Human Services (DHS). Sometimes these centers are located in facilities licensed by the Minnesota Department of Health (MDH) such as nursing homes or hospitals.

This agency serves adults 65 years old or older who are isolated, depressed, frail, cannot perform Activities of Daily Living (ADL), and/or is at risk of placement in a nursing home.
First, the person should come and visit to meet the staff and other clients and see if Hope Senior Center is a good fit. Second, based on physical and emotional needs, staff will advise you on the best way to contact his or her insurance company's care coordinator/case manager to complete a Long Term Care (LTC) Assessment and ask to participate at Hope Senior Center.
This service may be covered for persons with Medical Assistance Home and Community Based Care Waivers, Alternative Care Program, Veteran's Administration TRICARE or by Long Term Care Insurance Plans. Services may also be purchased private pay.
If you have waiver funding, the service plan (based on an assessment of your needs) determines which services are funded through your waiver.
Elderly Waiver (EW) accepted. For other insurance - contact your private insurance company's care coordinator. Private pay is also accepted.
Serve Hennepin, Anoka, Ramsey and Washington Counties.
The agency is in the city of Maplewood and will provide transportation and services to seniors who live within a ninety mile radius.
