Description

Day Training and Habilitation (DT&H) is a day program for adults with developmental disabilities that helps to:
* Learn and use life skills
* Participate in community life
* Choose activities to participate in that are personally meaningful


Services during the day:
* Services (or supports) for a person to be fully integrated into the community
The services help the person to participate in the community in activities that the person chooses.


Additional Information

Offers program to assist individuals to find and keep a job in their community


Developmental training
* For people with severe developmental disabilities or related conditions
* Provides community and contract work as well as activities in the community
* Provides support for communication, social and vocational skills and activities of daily life
* High staffing ratio to meet needs of the participants
* Five programs are located in Mankato as well as programs in Rosemount, Fairmont, New Ulm, and Chaska


Center based employment:
* Maintains this limited option for people who choose not to work in the community
* Workers are assigned to a MRCI Rehabilitation Counselor to assist with planning and maintaining their job
* Center staff will provide training and materials to workers to create the products
* Available in Mankato, Fairmont, New Ulm, Shakopee and Worthington


Personal Options Day Services (PODS):
* Offers supported employment program options to create an individualized plan
* Employment, recreation and education are offered to participants


Employment And Social Enrichment (EASE) Older Adults Program:
* Structured day program in Mankato for aging people who have disabilities
* Offers a combined work and leisure day program or leisure only program
* Programs are designed to each person's interests and desired activity level
* Activities include:
- Center based arts, crafts and educational activities
- Community outings to take in sporting events, shows, fairs or lunch
- Exercise and nutrition program


About This Location

Some DT&H services that may be available:
* Supervision, training and help in self-care, communication, socialization and behavior support
* Center based work opportunities
* Activities in the community, like an exercise class, volunteering or special events
* Training in community inclusion skills, money management and activities to live more independently
* Transportation services for non-medical purposes to enable persons to participate in DT&H services (DT&H transportation can only be used for DT&H services)

Eligibility

To be eligible for DT&H services through the state of Minnesota Disability Services, a person goes through a screening.


People who are eligible for this service meet four requirements:
* Are 18 years or older (including people who are aging)
* Are diagnosed with a developmental disability
* Receive:
- Home and Community Based Services under Medical Assistance (MA) or county contract; or
- Home and Community-Based Waiver (such as the Developmental Disabilities (DD) waiver); or
- Are living in a Intermediate Care Facility for Persons with Developmental Disabilities (ICF/DD)        
* Make an informed choice to get this service


Under Waiver Reimagine, day training & habilitation and structured day program are streamlining into one service called day support services as of January 1, 2021 (or upon federal approval).

Fees

For people who are screened to meet all four eligibility criteria, the Minnesota Department of Human Services (DHS) Disability Services through the county, Medical Assistance (MA) or Developmental Disabilities (DD) waiver pay for DT&H services. Private pay or other funding options may be available.
