This program provides non-emergency transportation for individuals who require routine medical services and who, because of their physical or mental conditions, are unable to use other available means of transportation.

Eligibility
Eligibility is determined by client's physician. Clients must live in Northwestern Minnesota.
Application Instructions
Services are initiated by a phone call from a client or representative. Client qualification is determined by medical number and state computer check. A certificate of need form from the physician may be required.
Once eligible, clients can schedule the ride. They are picked up at their door and delivered to the desk of the requested medical facility. Usually, one client is transported per vehicle or trip.
24 hours notice is preferred, but they will try to honor same day requests.

Also included are programs that provide transportation for youngsters with disabilities who have no other means of accessing necessary specialized services and activities. Some but not all vehicles used for this purpose are equipped with wheelchair lifts.

Programs that provide transportation for medical appointments, trips from and between hospitals, to nursing homes or other special-care centers or for other non-emergency medical needs for individuals who, because of financial problems or their physical condition, are unable to use other means of local transportation. NEMT service providers are able to transport riders who require a wheelchair lift equipped vehicle with appropriate protective restraints or a van that is designed for gurney/stretcher transportation, or has other types of features to meet their safety needs.
