Metropolitan Center for Independent Living (MCIL)
Nursing Home Relocation
530 Robert St N, St. Paul, MN, 55101-2210
Distance: 203 Miles
This program helps people, who have disabilities and are aged 18 - 65, move from nursing homes and group homes into less restrictive, more independent and affordable housing in the community. The program also helps clients find the medical and social services and supports needed to live independently, such as Personal Care Attendants, who can help with essential daily activities, like bathing, dressing and feeding.
Many people relocating from nursing homes have very few of the basic household items needed to start over again. The Transitional Services program can help with the transition to a new home by providing furniture and household items like bed sheets, towels, and cooking equipment, and can also arrange for delivery of these items to the new home.
If you qualify for the programs these services are provided at no cost.
Program is for nursing home residents between the ages of 18 and 64 who have a disability, are eligible for medical assistance, and no longer need the level of medical care provided at a skilled nursing facility.
Referrals from a county case worker.
Paid for through waivers or private pay.
