Eligibility
To be eligible for the Urgent Need program:
* Must be a resident of Minnesota, with proof of residency identification
* Have an urgent need for insulin
* Have a current insulin prescription
* Pay more than $75 each month for insulin
* Not be enrolled in Medical Assistance or Minnesota Care
To be eligible for the Continuing Need program:
* Must be a resident of Minnesota, with proof of residency identification
* Have a family income equal to or less than 400% of the federal poverty guidelines income
* Not be enrolled in Medical Assistance or Minnesota Care
* Not be eligible to receive health care through most federally funded programs (some eligibility may exist for Medicare Part D enrollees)
* Not be eligible to receive prescription drug benefits through the Department of Veterans Affairs
Application Instructions
For the Urgent Need program, fill out an application and bring the completed form to your pharmacy with a valid identification and insulin prescription.
For the Continuing Need program contact a trained MNsure navigator for application assistance.
Fees
Urgent Need program users can receive a 30-day insulin supply for no more than $35.
Continuing Need program users can receive a year supply of insulin for no more than $50 for each 90-day refill.
