Description

Moving Home Minnesota (MHM) helps people move from institutions to their own home in the community. MHM promotes the development and implementation of transition plans that reflect the preferences of people receiving services and the opportunity to receive services in their home in the community.


Lead agencies (counties, tribal nations and managed care organizations) will help the person receiving MHM services select a transition coordinator. The transition coordinator will help create and implement a plan to transition the person back to the community.


Moving Home Minnesota's objectives include:
* Assisting people with transitions from various institutions to community living
* Allowing states to develop, implement and evaluate demonstration and supplemental services not otherwise covered by the state's Home and Community-Based Services (HCBS) waivers or State Plan.
* Supporting Tribes and Tribal Organizations with resources to build a sustainable community-based Long-term Services and Supports (LTSS).


With MHM, you can move to:
* A home that is owned or leased by you or your family
* An apartment with a lease
* A Minnesota Department of Health licensed assisted living residence with a lease, or
* A home in a residential setting with four or fewer unrelated people living there.


With MHM, you can get help with:
* Planning your move
* Paying for moving expenses, deposits, equipment and home modifications
* Finding a safe and secure place to live
* Connecting with your community
* Accessing subsidized housing, if available

Eligibility

You may be eligible for MHM if you:
* Are a resident of Minnesota
* Are enrolled in Medical Assistance (MA)
* Have lived in an institution for 60 or more consecutive days
* Have an institutional Level of Care (LOC)


Qualified institutions may include:
* Hospitals, including Community Behavioral Health Hospitals (CBHHs)
* Nursing facilities
* Intermediate Care Facility for Persons with Developmental Disabilities (ICF/DD)
* Child and Adolescent Behavioral Health Services (CABHS)
* Psychiatric Residential Treatment Facilities
* Institutions for Mental Diseases (IMD) enrolled as a participant in the Minnesota Substance Use Disorder System Reform 1115 Demonstration.
