Program where staff or volunteers interact with people who live on the street, build relationships, try to identify immediate needs and provide information about longer-term support services.

Eligibility
Assists long-term unsheltered homeless adults who experience mental illness in Hennepin and Ramsey county. To be eligible for services, individuals must have been homeless for a year or longer OR have four episodes of homelessness in the past three years. Additionally, individuals must be unsheltered, meaning they are living in a place not meant for human habitation.
Application Instructions
Referrals to the street outreach program come through a variety of community partners, such as case managers, law enforcement, and other social service providers. Self-referrals are also accepted.

Fees
Cost is covered by a variety of grant resources as well as a Housing and Urban Development (HUD) services-only grant.
