Description

Meal delivery programs prepare and deliver nutritious meals to older adults and people with disabilities who are unable to make the food for themselves or travel to a meal site.


Additional Information

The Prairie Five Meals Program serves 24 communities with home delivered and frozen meals.


Frozen meals are available for weekend, holiday, emergency and rural delivery.


Special diets are available with a doctors order

When receiving this service under waiver programs, you can receive up to one home-delivered meal per day.

Eligibility
To receive this service under the Alternative Care (AC), Essential Community Supports (ECS) and Elderly Waiver (EW) programs:
* The person must have an assessed need for the service that is documented in their support plan
* The service must be the most cost-effective method to provide the person with a nutritionally balanced meal

To receive this service under the Brain Injury (BI), Community Access for Disability Inclusion (CADI), Community Alternative Care (CAC) and Developmental Disabilities (DD) waivers:
* The person must be age 18 or older
* The person must be unable to prepare their own meals due to their disability
* The person must have an assessed need for the service that is documented in their support plan
* The service must be the most cost-effective method to provide the person with a nutritionally balanced meal
* No one else is available to prepare the meals

If you have waiver funding, the support plan (based on an assessment of your needs) determines which services are funded through your waiver.

Application Instructions

Call the dining site to reserve a meal or for home delivery.


Fees

Suggested donation rate is $4.50 for ages 60+, $8 for those under 60.
Children under 12 pay $4 and receive 1/2 portions.


Food Stamps (Supplemental Nutrition Assistance Program (SNAP) vouchers) accepted
