Housing for people with low income that includes coordination of other services for residents. The coordination professional can help residents identify resources, navigate systems or help with applying to additional programs.


Additional Information

Harvest Hills is a permanent supportive housing program for eight families in Anoka County. It offers residents on-site case management support.


Housing Opportunities for People to Excel (HOPE) Harbor is a supportive housing complex in Minneapolis for low-income single adults and the chronically homeless.
Services include:
* Social workers/case managers available
* Assistance with self-sufficiency skills, employment, medical/dental referrals, recreation, chemical dependency referrals, mental health services, educational services, information and referral


The Foyer Housing Program at the Booth Brown House offers permanent supporting housing to young people age 18 - 24, with the support and resources necessary to make the transition to independence.

Eligibility

The Harvest Hills program supplies permanent-supportive housing in Anoka County for previously-homeless families. It serves a vulnerable population who struggle to afford rent and have at least one family member living with health issues.


To qualify for the Foyer Housing Programs, youth must be:
* Between the ages of 16 - 24
* Homeless or at risk of homelessness
* Transitioning from a public system, such as foster care, residential treatment centers, or homeless shelters
* In need of extra support to transition to a safe and stable adulthood
