Offers non-emergency medical transportation. Vehicles are equipped ramps and lifts.

* Common Carrier Transportation for people whom can transfer into a vehicle without assistance

Includes one vehicle with a stretcher in Mille Lacs, Sherburne, Benton and Wright counties
Will pick-up from Benton, Cass, Mille Lacs, Sherburne, Wright counties and transport to anywhere in the state
Application Instructions
Pre-approval is required through the Medicaid insurance company and documentation needed for Medicaid appointment over a 30, 60 mileage.
Transportation available Monday - Saturday, flexible times if pre-scheduled. Transportation hours: 4:00am - 6:00pm.

Accepts Medicaid plans delivered through Blue Cross/Blue Shield Health Partners, Medica and UCare

Programs that provide transportation for medical appointments, trips from and between hospitals, to nursing homes or other special-care centers or for other non-emergency medical needs for individuals who, because of financial problems or their physical condition, are unable to use other means of local transportation. NEMT service providers are able to transport riders who require a wheelchair lift equipped vehicle with appropriate protective restraints or a van that is designed for gurney/stretcher transportation, or has other types of features to meet their safety needs.
