Description

Meal delivery programs prepare and deliver nutritious meals to older adults and people with disabilities who are unable to make the food for themselves or travel to a meal site.


Additional Information

Noon meals are delivered to the home by volunteers

When receiving this service under waiver programs, you can receive up to one home-delivered meal per day.

Eligibility
To receive this service under the Alternative Care (AC), Essential Community Supports (ECS) and Elderly Waiver (EW) programs:
* The person must have an assessed need for the service that is documented in their support plan
* The service must be the most cost-effective method to provide the person with a nutritionally balanced meal

To receive this service under the Brain Injury (BI), Community Access for Disability Inclusion (CADI), Community Alternative Care (CAC) and Developmental Disabilities (DD) waivers:
* The person must be age 18 or older
* The person must be unable to prepare their own meals due to their disability
* The person must have an assessed need for the service that is documented in their support plan
* The service must be the most cost-effective method to provide the person with a nutritionally balanced meal
* No one else is available to prepare the meals

If you have waiver funding, the support plan (based on an assessment of your needs) determines which services are funded through your waiver.

Eligibility

Anyone age 60 and older and confined to their home or unable to prepare a balanced meal. Spouses are automatically included regardless of age.


Serves Dodge, Fillmore, Freeborn, Houston, Mower, Olmsted, Steele, and Winona counties


Application Instructions

Call the local site between 9:00am - 12:00pm to reserve a meal.


Fees

Donations are accepted for meals. No one is denied a meal due to inability to contribute.
