Description
Medicare Partners (formerly known as Senior Partners Care) is a membership program (not insurance) made possible through a partnership between the Federation and specific local health care providers.
It can help lower out-of-pocket costs (co-payments and deductibles) for enrolled members with Medicare Parts A and B who meet specific income guidelines.
Keep in mind that:
* Medicare Partners is NOT insurance
* It is only "good" at providers who have agreed to participate in the Medicare Partners program. Not all local physicians, clinics or hospitals participate, and they do so voluntarily.
* Medicare Partners is not a payer of bills, nor do they guarantee payment. Medicare Partners is simply an agreement with the provider to consider waiving your Medicare coinsurance and deductibles.
* Medicare Partners only applies to Medicare-covered services. If Medicare does not cover the treatment or item you receive, you will be responsible for the balance. Examples include dental care, eyeglasses, refraction and hearing aids.

Eligibility
* Must be enrolled in both Medicare Part A (hospital) and B (medical)
* Must meet annual income and asset guidelines
* Must not have additional Medicare health insurance coverage for doctor or hospital services
Medicare Partners is a statewide program and Seven County Senior Federation is the Administrative Entity.

Call to get plan information at (866) 679-4700, Monday - Friday, 9:00am - 4:30pm.
Complete the application and provide adequate financial information to confirm proof of eligibility before you will be accepted into the Medicare Partners program.
