Description
Mental Health Case Managers coordinate services, design treatment plans and plan for crises to serve people of all ages with mental illness.
Plans for mental health services are customized for each person's needs and designed to help people live with as much independence within the community as possible.
Additional Information
Adult Mental Health Targeted Case Management services help adults with a "serious and persistent mental illness" gain access to needed medical, social, educational, vocational, financial and other necessary services as they relate to the recipient's mental health needs. Once eligibility is determined, case management may be provided by a county case manager or through a contracted agency.
Children Mental Health case management is a voluntary service provided at the request of a family whose primary concern is the unmet mental health needs of their child. A case manager assists the family in obtaining appropriate community resources and services for their child and works with other providers to coordinate the care for the child.
