Outpatient health clinic offering preventative, diagnostic (exam or test) and treatment services

Some clinics are certified as Health Care Homes, where primary care providers, families and patients work together to support wellness and improve health outcomes. Clinics with the Health Care Homes certification can be found online.

Basic health care offered at a medical clinic, to low income, homeless or uninsured populations

Note there are no walk-in services

Medical clinics that provide free medical care, outreach and education to low-income, uninsured families and individuals who are not eligible for government programs.

Care includes:
* A patient's visit to the clinic
* Lab tests
* X-rays
* Diagnostic tests
* Access to most medications
* Mental health
* Preventive medical care and for certain chronic conditions such as diabetes, high blood pressure, asthma, and rheumatoid arthritis
* Interpreters

Specialty referrals (when needed) are given to a patient without charge through coordinated efforts with area health care providers.

Clinics are located in spaces donated by churches, schools, and community centers in the metropolitan area. Directions to the closest clinic will be given when an appointment is made.

Care not provided for:
* Dental
* Obstetrical (pregnancy)
* Family planning
* Fractures
* Emergency type services
* Complicated chronic conditions

Staff can assist qualified patients in applying for MNsure and state programs such as MinnesotaCare and Medical Assistance (MA) to support long term health care access
