Description
Consumer Directed Community Supports (CDCS) Support Planner (formerly known as Support Broker/Flexible Case Manager) can provide support planning services to help a person develop an individual support plan and budget for buying in-home services and/or supports.
These personalized supports help a person continue living at home and help avoid or delay out-of-home placement.
The CDCS Support Planner service is based upon a person-centered planning approach, where the support planner helps the person develop and implement a self-directed support plan to receive needed services and supports.
CDCS Support Planner services may include helping people:
* Select, employ, train, schedule, and monitor their workers
* Define worker responsibilities and tasks
* Evaluate and monitor results
* Create and modify their plan and budget
* Problem solve
* Coordinate service delivery
The person and/or family caregiver determines how much help they need from the CDCS Support Planner service.

Eligibility
Any person seeking help in planning and developing services and supports to remain at home using an established budget either from public or private funds. Public funding is contingent on the availability of state and federal funding.

Programs that help older adults or people with disabilities and their caregivers develop a personal plan and budget for purchasing in-home services and/or supports that will help them avoid premature institutionalization. Support brokers help people select, employ, train and schedule personal care attendants and other support providers; define worker responsibilities and tasks; evaluate and monitor results; advocate and problem solve, as required; coordinate service delivery; and arrange for other services as needed by the person to remain at home. Support brokers monitor the individual's budget and represent the person when dealing with service providers but do not provide direct supportive living services.
