Description
Case managers help people to plan, access, evaluate and coordinate services from their service providers. Plans are customized to each person's own needs and preferences.

Information, referral and case management to residents in Minneapolis Public Housing Authority (MPHA) apartment buildings.
Provides case management, information and referral and group services to residents age 60 and older and/or are disabled
Assistance includes:
* Economic assistance eligibility (medical assistance, food stamps and cash assistance)
* Physical and mental health
* Financial concerns
* Housing needs
* Personal crises
Staff have access to a translation service to meet language needs of all residents. Bilingual Somali staff are available to meet language and cultural needs of Somali residents.
